Black out / remove student names and personal identifiers.

Access DATA SUBMISSION FORM
*Only one subject/ grade/ class can be reported at a time. Repeat process to submit data for additional subject(s)/ grade(s)/ class(es).
4.
Input Assessment Type:
District Created / Compiled - or - School Created / Compiled
5.
Input Assessment Scale:
*Inquire at the school level, if support is needed.
6.
Input Class Average:
*Inquire at the school level, if support is needed.
7.
Upload Class Report without student names or personal identifiers.
8.
Contact your MTLD for additional assistance.
